Cara L. Jakob, MD
Dr. Jakob is a board certified family physician. She completed her undergraduate studies at the University of Connecticut and earned her medical degree from the Drexel University School of Medicine in Philadelphia, Pennsylvania. She completed her residency training at Tallahassee Memorial Regional Medical Center. After moving to Clermont, Florida, Dr. Jakob opened Total Family Healthcare & Wellness Centers where she now practices as a family physician and medical director.
What makes Dr. Jakob’s practice unique is her approach to medicine. At Total Family Healthcare, disease prevention and patient quality of life are the cornerstones of her practice. Dr. Jakob also offers a comprehensive weight loss program, immigration physicals and Suboxone® treatments for substance abuse.
Stacey Rivera, ARNP
Stacey Rivera ARNP, FNP-BC is a board certified Family Nurse Practitioner. She completed her undergraduate as well as her graduate studies at the University of Central Florida, where she graduated Sigma Theta Tau with honors. Stacey implemented the Chronic Care Management program for the office, which has a focus on continuity of care and reducing hospital readmissions. As a Nurse Practitioner her focus is a holistic approach to healthcare in which the whole person is treated with an emphasis on patient education and prevention.
Brandon M. Crotty, LMHC, CAP
Brandom M. Crotty, LMHC, CAP is a Licensed Mental Health Counselor and Certified Addictions Professional. He completed undergraduate and graduate studies at Canisius in Buffalo, NY. he has provided therapy to individuals and families in educational and clinical settings for 20 years. Much of his current efforts are dedicated to the field of Substance Abuse and Opioid Dependence. He incorporates the entire family dynamic within the recovery process. Educating family and loved ones about the specifics of addiction remains a vital component of his continued care.
