Dr. Jakob is a board-certified family physician and founder of Total Family Healthcare in Clermont, Florida. She completed her undergraduate studies at the University of Connecticut, earned her medical degree from Drexel University, and completed residency at Tallahassee Memorial Regional Medical Center. Her practice emphasizes disease prevention and patient-centered care, and was among the first to implement fully integrated electronic medical records. As Medical Director, she also provides treatment for substance abuse.
Dr. Jakob co-founded two physician-owned accountable care organizations—Primary Partners and Primary Partners ACIP—pioneering value-based care in Florida. As Clinical Integration Director, she collaborates with hospitals, long-term care facilities, and community agencies to improve care coordination, reduce costs, and enhance patient outcomes.
She chairs the Patient Integration and Transition of Health Committee, helping to develop care protocols across settings including hospitals, ALFs, and home care. A passionate educator, Dr. Jakob serves as a preceptor for several Florida universities and has received multiple teaching awards for her contributions to medical education and innovation.
